Go online by a search of covid jab, do the little question and answer, it will tell you what group your in. If your in 1a or 1b you can go to the next step of booking a time to have it. Thats if you can actually get onto the ones doing the vaxinations. I gave up and went online to the local hospital. Not well known that they are doing it. Plenty of times free for days. Booked one and had it done. Or you wait till your pharmacy gets around to doing it.
What personal information do they want? Is it extensive or not like the flu jab - nothing required.

As a person in the 1b (70 to 79 year age), I had to ask to receive the Covid-19 vaccine, prior to it coming to my area This meant that I had to give my personal information
I read, in the past, that anyone who has the Civid-19 vaccination, will be on a master list I would assume that the master list, will have my personal information
When I had the vaccine, I received a paper with the following info My name, my date of birth, date of Dose: 1, Batch lot # of the vaccine, expiry date of the vaccine, and second dose appointment date
This paper also had what to expect after the vaccine, etc, etc
I have no idea what will happen in the future, but I do doubt, that a person could walk in off the street, with no identification, and receive the vaccine

You will need Medicare card only if attending regular surgery, plus ID document/s if not a regular at surgery.
When you go in they will require you to fill out a three page tick and flick questionnaire regarding health.

Sorry Brodie, I am none the wiser about 'the' jab however, when it comes to the flu jab, I have been getting it mainly at the chemist that looks like a warehouse for years now. I go on-line, book and answer heaps of questions but mostly, apart from name, address ans Medicare number, they are just tick boxes. You also have to sign at the end so your signature looks more like scribble, but easy as. It would be good if 'the' jab was that easy. Like you, I don't mind waiting.
I am still in the habit of avoiding crowds where possible and keep safe distance and sanitize often, daily. My hands get more alcohol than my gut.
