Forms available on-line or at a post office. Used them when we shifted house.

Pass The Post is a company we've been using for over 5 years. We redirect our mail to them they hold it till we tell them where to forward it to, which is ussually a Post Office that we'll pass in our travels.  Or you can can get them to scan the mail and email it to you.  For us they are great as we have mail and parcels on a regular basis.

Hope this helps, regards Steve.
