why do caravans part from the tug

Hi. A silly question maybe, but I have to ask.

What are the different causes that result in a caravan coming adrift from the tug?

And what would be the most common cause, in your opinion.

I am doing some research and appreciate any help.

IMO from top to bottom,
Poorly loaded,
beyond the capability of the tug,
poorly maintained running gear,
outside forces (which includes road conditions),
inexperienced operators who react with incorrect righting/handling procedures
Sh%# happens..
